Tesamorelin: The Stabilized GHRH Analog with Clinical Trial Data
Last updated June 2026 · Clean Reference Format
Summary
Tesamorelin (TH9507) is a 44-amino acid stabilized analog of growth hormone-releasing hormone (GHRH). The N-terminal trans-3-hexenoic acid modification confers resistance to DPP-IV enzymatic degradation, extending functional half-life beyond that of unmodified analogs like Sermorelin. It has completed Phase 3 clinical trials with statistically significant reductions in visceral adipose tissue. It acts on the GHRH receptor on anterior pituitary somatotrophs, stimulating physiologically pulsatile GH release.
Mechanism
Tesamorelin binds the GHRH receptor (GHRH-R) on pituitary somatotrophs → activates adenylyl cyclase → increases intracellular cAMP → stimulates GH gene transcription and pulsatile GH release into circulation. Released GH drives hepatic IGF-1 production, promoting lipolysis — particularly in visceral adipose depots.
Key Data
- Phase 3 trials (2 mg/day SC) across 800+ subjects demonstrated ~15–20% median reduction in VAT by CT imaging over 26 weeks.
- Effects were visceral fat-specific — subcutaneous fat was not correspondingly increased.
- Improvements in waist circumference, trunk fat, and hepatic fat content were also reported.
- VAT reductions reversed upon discontinuation, consistent with an ongoing-signaling-dependent mechanism.
